Instructions for Downloading Our Patient Forms
& Patient Questionnaire

Please feel free to download our forms to conveniently fill out at home.

1. Download the forms by clicking here or by clicking the "Download Forms" button at the bottom of the page. You can download the The forms should open up automatically in your browser. If it does not, save the file to your computer.
2. Once downloaded, open up the file. If you can not open the file, "tmj_forms.pdf" or "patient_questionnaire.pdf", it means that you do not have Acrobat Reader installed on your computer. Please click here to or the button below to download Acrobat Reader.
3. After you have opened the "tmj_forms.pdf" or "patient_questionnaire.pdf", you can fill the form out by typing in your answers or you can print the forms and fill them out with pen.
4. Print the forms by either clicking the print button at the top of the browser or clicking the print button on the last page of the form.
5. After filling out and signing the forms, you can give them to us on your next visit or send them to our office address:
